Clearance from Medical Expert on Physical Readiness
ZVPD WELLNESS LLP (nor any of its employees, agents or representatives), is engaged in rendering medical advice, nor does it (or any of its employees, agents or representatives) hold itself as qualified to do so. The content of our website/ products/ services is not intended to replace or be a substitute for professional medical advice, professional dieticians advice, diagnosis or treatment and has been created for information purposes only.
We strongly recommend that you seek professional medical advice before embarking on any diet or exercise program.
Many health benefits are associated with regular exercise and the completion of a 'Physical Activity Readiness Questionnaire' (PAR-Q) is a sensible first step to take if you are planning to increase the amount of physical exercise in your life. For most people, physical activity should not pose any problem or hazard. The PAR-Q is designed to identify the small number of adults for whom physical activity might be inappropriate or those who should seek medical advice concerning the type of activity most suitable for them.
Accordingly, please read the following questions carefully and answer each one honestly.
1. Do you have a bone or joint problem such as arthritis, which has been aggravated by exercise or might be made worse with exercise?
2. Do you have high blood pressure?
3. Do you have low blood pressure?
4. Do you have Diabetes mellitus or any other metabolic disorder?
5. Has your doctor ever said that you have raised cholesterol (serum level above 6.2mmol/L)?
6. Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
7. Have you ever felt pain in your chest when you do physical exercise?
8. In the past month, have you had chest pain when you were not doing physical activity?
9. Is your doctor currently prescribing you any drugs or medication?
10. Have you ever suffered from shortness of breath at rest or with mild exercise?
11. Is there any history of Coronary Heart Disease within your family?
12. Do you ever feel faint, have spells of dizziness or have ever lost consciousness?
13. Do you currently drink more than the average amount of alcohol per week? [Average Amount: 7 pints of Beer 'OR' 7 small glasses of Wine 'OR' 25 ML measures of Spirits]
14. Do you currently smoke more than 4 cigarettes a day?
15. Are you, or is there any possibility that you might be pregnant?
16. Have you given birth to a child within the last 2 months?
17. Have you had any recent surgery?
18. Do you know of any other reason why you should not participate in a programme of physical activity?
If you answered YES to one or more questions in the PAR-Q, consult with your doctor by telephone or in person before increasing your physical activity. Inform your doctor of the questions that you answered ‘yes’ to on the PAR-Q. After medical evaluation, seek advice from your doctor as to your suitability for unrestricted physical activity.
If you answered NO to all questions in the PAR-Q honestly and accurately, you have reasonable assurance of your present suitability for a graduated exercise programme.

You hereby state and agree that you have read, understood and answered honestly the questions in the PAR-Q. You also state that you wish to participate in Stronger with Zoe programs which involve activities, which may include aerobic exercise, resistance exercise and stretching.
